To lodge a claim, use the Football NSW Claims Portal. Follow these steps for a smoother process:
Complete the Claim Online
Register and fill out the form on the claims portal.
Download and print the Club Declaration and Physician’s Statement.
Complete Required Forms
The Club Declaration is filled out by your club, saved, and uploaded to the portal.
The Physician’s Statement is completed by your doctor and uploaded.
Submit Supporting Documents
Ensure all necessary documents and receipts are uploaded to expedite processing.
Claim Verification
Claims are verified by either your Association or Football NSW to confirm registration, activity, and sanctioning.
